Early Childhood Initiative
ECI is a voluntary home visiting program in Madison/Dane County, WI for pregnant women and families with children aged 0-4K. ECI knows that caregivers are the experts on their families and supports them in working towards their goals for parenting, health and wellness, relationships, school and work, and resource assistance.
Children Come First
The Children Come First (CCF) program at Community Partnerships is a program for children who are living with chronic and persistent mental health issues. CCF coordinates mental health services, community resources, and other needed services to support children and their families by using a strengths-based approach to wraparound care coordination.
Transition to Independence
The Transition to Independence Program (TIP) is a program that supports mental health recovery in young adults, ages 18-30, through an individualized and strengths-based approach to care coordination. TIP participants are offered the opportunity to set their own goals for mental health and wellness, independent living, and peer support.
Court Ordered Evaluations
The COE Program utilizes a diverse provider network of over 40 qualified mental health professionals including Master’s level clinicians, Licensed Psychologists, and Licensed Psychiatrists to provide objective assessments and evaluations to Dane County court for the purposes of treatment planning and placement decisions.
Comprehensive Community Services
Comprehensive Community Services (CCS) is a Medical Assistance program administered by Dane County Human Services (DCHS) to provide a flexible array of individualized, community-based, psychosocial rehabilitation services to individuals in Dane County with mental health and/or substance use issues across the lifespan.
Community Partnerships is a non-profit leader in Madison, WI for early childhood, coordinated care, mental health, and wellness services. Our services provide support, education, and connection to community resources so that children, families, and individuals may remain in their homes and community.
